Lifestyle
Personal care is our specialty. Our goal is to improve our residents' well-being and safety by providing an environment in which they can thrive. We provide three nourishing meals daily, snacks, housekeeping, activities and 24-hour nursing supervision. Our trained staff will assist with daily living activities for those residents who need assistance with personal hygiene walking, eating and supervision of medication.
Assisted Living
For those who may need assistance with bathing, dressing, medication supervision or grooming, our Assisted Living is just what you need. Our twenty four hour supervised assisted living area offers trained support staff that are available to assist you in medications, bathing, laundry and dressing. Fear of health issues are readily addressed through our Nursing and Resident Assistant personnel. Continue to reside in a spacious apartment with privacy and security with daily activities and outings to keep you busy and socializing with friends. If the need arises for rehabilitation including physical, occupational or speech therapy, The Plaza at Pembroke Park provides each person with specific and targeted therapy to assist them to reach their highest physical potential. Our therapy services program Is offered seven days per week. This provides the momentum to recover from any significant health event. We offer care plans through an interdisciplinary team including the resident, family, physician and staff, creating an excellent synergy for recovery and overall wellness.
Memory Care
For those seniors who need special assistance due to memory impairment issues, The Plaza at Pembroke Park Memory Care program offers a secured, safe environment and outdoor courtyard for residents to enjoy. Staff members that work with residents in the Memory Care Program receive a high level of professional training in validating the life experiences of every individual.

reviewed on: 01/24/2017 by AnonymousA nice tour with a lot of great qualities for the care of a loved one. the staff was very helpful to answer all of the questions we had for the care of my mom. We would recommend other families to see this community and see that this is nice and taken care of.
This community was there when we needed a place for mom to continue to recuperate!!

Some of the staff seem to have an attitude and some are very cheerful. This community feels like a nursing home setting as most of the residents have memory issues and she felt isolated as she had limited interaction with the other residents.

The staff is amazing they are very helpful when I need some help. There are nice activities and good food offered. This is a community I would recommend.
Lots of Activities!
reviewed on: 01/17/2014 by LMBMy mother has an ostomy bag and they have to change that, and we were fortunate to find a place that had licensing. My mother doesn't utilize the activities, she says that she doesn't have anything to do but I KNOW they have a great activity program. BINGO, bus rides, singers come in. At Christmastime, I went to their Christmas party, and they had a wonderful buffet, a singer, conga line and limbo line! They have a lot for them, whether the people opt to do it is up to them! Anytime that there is an issue, it's resolved. When we were down there, it didn't seem like they dusted. I saw a lot of dust and I went down to complain, and they got back to me to say that they have tried to do that but mother is claiming she doesn't want anyone to move her things. We went to the community room to play card games with her, and we noticed that someone had been in her room and they had cleaned the dresser. I can't really say anything bad because if an issue comes up, they do rectify it.
